I use them and I wouldn't go back to an ordinary breast collar.
They fee up the shoulder, reducing strain, they free up the windpipe!
I noticed the difference of my horses way of going as soon as I switched over. They tired less, they could extend more freely. They are especialy useful for shetlands or horses with a narrow line of draft.
They are a product that a lot of research has gone in to and they make the normal breast collars seem numb and not thought out very well.

def not go back to an ordinary breast collar - Taz has quite a low head carriage and shallow chest , the empathy frees up her shoulder and gives neck lots room too.
we love it

Might be an idea for the Shetlands for exercise.
They have so little room for manouvre, little room between wind pipe and shoulder.

There is a better design though "Campton harness". This one called the Freedom collar..is better for both low and normal draft.
The Empthy collare works best on only the low draft. And does not work at all well with the smaller Shetlands and mini's.
I bought an Freedom collar..awhile back and hope to be able to try it out soon. I know it will help Maggie pull better.
If anyone is interested..here is the link
