Where do you find the vests that say horse and rider on the back? (I am in US, and have yet to find one!)

You could look on ebay as they have loads for sale in uk. They usually say 'caution horse and rider' In the uk it is often called a hi viz tabbard. I don't know what you would call it in usa. (although we speak the same language!) Good luck.

I personally get builder tabbards as they are cheaper, more durable and i think more comfortable if it just the simple reflective/flourescent tabbard you want. I prefere the jacket types to the ones that go over the head...

Yes the ones that go over your head are a real pain once you have your riding hat on. Myself and my daughter just have the cheap builders type ones that go on like a waistcoat and velcro at the front, the type with writing on seem a bit expensive.

Point North fabrics (http://www.profabrics.co.uk/index.php)have got a good selection of fluorescent and reflective tapes, fabrics and other bits and pieces if you want to add safety strips to your and your horse's existing clothing. I'm sure there are plenty of other suppliers of such items, too.
