How do you get a picture icon added to your name in the left column of the page?

Go to your user CP and look at the bar on the left. There should be a button saying Edit Avatar.
You then just upload a picture - it will tell you how. But you need to have a tiny picture - 50x50 pixels maximum I believe. So you many need to resize one first.
