Change of ownership, so how do i get it changed on her passport ?

Well i want to get my horses passport changed so shes under my name. But ive looked on the Weatherbys website but cant find anythin about it, can someone please give me some in sight, as i no im just bein dim again
Also is there a certain age you have to be to have it put under your name ?
