If she was mine I'd want the vet out to do an assessment and x-rays of the front feet. To me her front feet look long at the toe and too much out in front of her, I suspect x-rays would show a balancing trim is needed to line the hoof wall up with the pedal bone and that may bring some instant relief - it's a common problem with metabolic ones that a foot may look to have acceptable balance on the outside but x-rays show a different story. She's ok on the soft but not the hard so I'd also want sole depth and rotation looked at. Without this information I don't think you know what's going on, what can be done to help or if you're at a point of no return - the fact she's still willing to move makes me think there are things you can do to help (unless she's very heavily buted in the clip).
